Evaluation - this is the new one. Most of the GAP Diagnosticians should download, print and replace the copy in their book. The scoring sheet no longer exists!
Acceptance Form1 - All dog accepted into the GAP Program must have these 2 forms filled out and sent to us.

Owner Release Form - All dog accepted into the GAP Program must have one of these forms filled out and sent to us.
